FINANCIAL POLICIES & INSURANCE

Our primary goal is your health and well-being. We base our treatment and treatment decisions on you, your condition, and the management of your healthcare. We collect the fee for services at the beginning of treatment rendered based on our fee schedule. For your convenience, we will provide you with the needed documents for services rendered that you will need to submit to your insurance company for possible reimbursement that will be sent directly to the patient. Many of the procedures/treatments that Dr. Bloxham provides can be billed to your medical insurance. Please note that we are an out-of-network provider for all insurance plans and we are not Medicare providers.

At Columbia Center for Sleep Apnea & TMJ, we understand that navigating insurance can be difficult and overwhelming. We offer various payment methods, including all major credit cards, cash, checks, and Cherry financing. Payment plans and financial arrangements can be made on a case-by-case basis. We openly invite you to discuss all payment options with our staff prior to your initial appointment.

Some insurance companies do cover oral devices for sleep apnea if they determine it to be medically necessary. To be sure, it is suggested that you call your insurance company to determine if it is a covered service and how much you might expect to be reimbursed.
